Creating a problem description, assembling evidence, and putting the approved solutions into practice are frequently the first steps in this process. While effectively implementing health promotion interventions, nurses should utilize appropriate leadership, cooperation, communication, and change management methods during this process.

Health promotion requires change management, collaboration with patients and the general population, and consideration of priority health issues to achieve desired objectives. Positive patient outcomes and improved organizational procedures in the healthcare industry can be achieved through this collaborative approach. Reviewing available data to identify the most effective actions and strategies is essential for the success of such initiatives.

This practicum project focuses on Abi, a 30-year-old nurse who struggles with anxiety and depression. By improving the management of her condition, the project aims to enhance her overall health outcomes. This assessment introduces the health issue, highlights the need for evidence-based management strategies, and underscores the importance of leadership, communication, and change management in achieving sustainable improvements in patient wellbeing.
